I had this happen once, I think Your file name contains too many spaces. Replace them with underscores.

[img]my file name.jpg

should be [img]my_file_name.jpg

The forum will accept a few spaces, though.

Did that fix it? If you are trying to use it as an avitar, long addresses will not work.
